How to Apply to the Long-Term Care Facilities?
Applications to any long-term care facility in Ontario can be filed through your local Community Care Access Centres (CCAC). The CCAC will give instructions on how to fill in the application. The applicant can indicate up to three facilities of his or her preference on the application. The completed applications will then be processed by the CCAC. Once the CCAC has approved the eligibility of the applicant, this individual will be included on the waiting list of his or her chosen facilities. The CCAC will notify the applicant or his or her family once a placement is available.

Who is eligible?
According to the Ontario Ministry of Health, to be eligible, you have to meet the following criteria:
• 18 years and over
• Have a valid OHIP card
• Require 24-hour onsite nursing care
• Require assistance at daily activities
• Require frequent monitoring throughout the day for safety and well-being

What are the rates at Mon Sheong?
The rates we charged are set up by the Ministry of Health and Long Term Care.

The fee includes accommodation, meals, nursing and personal care, housekeeping, laundry and activity programs.
