Position Summary
Under the direction of the Senior Administrator or designate, incumbent assumes the authority and responsibility for the nutritional care for the residents and takes on the role to monitor, oversee the day to day clinical nutrition of residents and maintaining a high level of nutrition care services and well-being of residents. Incumbent will be required to travel to assist with duties at Downtown, Scarborough and Richmond Hill Long Term Care facilities.

 

Main Duties
– Oversee menu planning process and prepare cycle menus to meet requirements in the the Fixing Long-Term Care Act, 2021 including:
o A minimum of 21-day menu cycle for regular therapeutic and texture modified diets and snacks.
o Standardized recipes and nutrient analysis for each menu item.
o Ensuring planned alternative menu choices for entrees, vegetables, and desserts and menu substitutions are of comparable nutritional value as the first choice.
– Participate in reviewing current and new systems, program, policies and procedures.
– Ensure that the nutrition and hydration care programs comply with the Fixing Long-Term Care Act, 2021.
– Reassesses residents’ nutrition care following RAI-MDS schedule or as needed.
– Assess new admissions within 21days and develop resident-centered nutrition care plans congruent with residents’ needs and rights.
– Attend the multidisciplinary care conferences to discuss the nutritional aspects of individual residents and revises the nutritional plan as necessary.
– Advise the health care team to review and identify nutrient related health problems and determine their treatment goals.
– Participate in the annual evaluation of the medication management program.
– Any other duties as assigned.

 

Qualifications
– Must be eligible or already registered with the College of Dietitians of Ontario.
– Three years previous professional experience in long-term care.
– Excellent leadership, communication and interpersonal skills with an interest in gerontology.
– Self-starter who is professional, energetic, and resourceful.
– Sensitive to residents’ cultural, spiritual, and physical needs.
