
Position Functions:
The RAI Coordinator serves as a dedicated role to review the interdisciplinary and comprehensive resident assessments, collecting data for the purpose of quality documentation management and adherence to the Minimum Data Set (MDS) and Resident Assessment Set (MDS) and Resident Assessment Instrument (RAI) requirements, providing support to the nursing and interdisciplinary team.
Responsibilities:
- Responsible for managing the coordination and completion of the Resident Assessment, Resident Assessment, Resident Assessment Protocols (RAPs) and care plan as assigned.
- Complies with Ministry of Long-term Care regulations regarding completion of the interRAI-MDS process
- Completes accurate coding of interRAI-MDS and care plan documentation for all residents.
- Continually updating knowledge base related to data entry and computer technology.
- Chair RAP chat as required.
- Attend clinical team meetings as needed and available.
- Any other duties as assigned.
Qualifications:
- Registered Practical Nurse with a current certificate of performance and in good standing with the College of Nurses of Ontario.
- Possession of valid CPR, and First Aid Certificates.
- 3- 5 years’ experience in long term care, experiences with RAI-MDS is required.
- Current RAI-MDS certification and Relias certification is required.
- Strong understanding of Point Click Care and RAI-MDS.
- Demonstrated supervisory and team leadership skills.
- Clear aptitude for detail-oriented work.
- Adherence to professional competencies, i.e. caring and respectful behaviour, clinical knowledge and skills, assessment skills, communication skills, conflict management skills and organization skills.
- Flexibility to work variable shifts, self-directed and able to function in a fast-paced environment.
- Proficient computer literacy and strong problem-solving abilities with excellent time management
- Proficiency in oral and written English; ability to speak Cantonese/Mandarin is an asset.
